Each year the Children’s Heart Foundation Ohio Chapter presents the following awards. If you would like to nominate an Ohio resident for an award, please complete one of the forms below and submit to [email protected].
Loving Heart Award: Given to a medical or services professional who demonstrates great compassion in caring for families impacted by congenital heart disease. This person can be a nurse, doctor, social worker, child life worker, hospital volunteer, or anyone who demonstrates great care and concern for patients and families in a health care setting.
Mending Heart Award: Given to a physician that has made an impact on a patient and family of congenital heart disease. This person can be a surgeon or cardiologist that has shown compassion and comfort to the family at its most vulnerable time.
Smart Heart Award: Given to a person who works in the medical community that makes an impact on congenital heart disease through research and/or medical advancement. This person has shown dedication by focusing their efforts of improving outcomes and improving medical therapies for congenital heart patients.
